Long Form Quote Request When you complete the long form quote request you will be asked to provide additional information to that provided for the short form request namely, your home address, structured settlement or other annuity payment dates and amounts, insurance issuer name, if you have received any other quotes and if so from whom and in what amount. You are asked for this information so that we may properly prepare an offer to buy your annuity payment(s). Should you accept our offer there is also sufficient information for us to arrange for the preparation and delivery to you of sale documents either by us or our designated processing service.
